I am a qualified Community Health Nurse, working under Ghana Health Services for over 5 years till date. In my line of duty i have had so many experiences in; Health promotion, Disease prevention, Materna and Child health care, Domiciliary Home visit and care of aged and disabled, Promote patient's independence by establishing patient care goals, teaching patient, friends, and family to understand condition, medications, and self- care skills. -Monitor clients’ physical and mental health reporting concerns to management for swift action. -Assisting patients in activities of daily living e.g. aide patients with bed routine, showering, dressing and feeding. -Assessing,planning, implementing nursing interventions and evaluating patient’soutcomes. -Providing comprehensive instruction on discharge plans to sustain continuity of care upon discharge. -Demonstrating effective leadership and encouraging team work, while appreciating team dynamics and redirecting focus to patient health care needs.
